VisitationWe have open visitation at Ashtabula County Nursing and Rehabilitation Center. We ask that visitors be mindful of the time of day and the Residents' personal schedule and/or tolerance. In order to safeguard our residents, we ask that all visitors leave by 8 PM, the same time our doors lock for the night. If your loved one is critical, we will arrange to accommodate your hours.SmokingSmoking is only premitted outside or inside within our smoking room during the designated smoking times. No smoking material is allowed to be in the resident's room. Instead, all smoking items will be locked in the smoke room when the resident is not smoking.Personal BelongingsA resident is encouraged to bring their personal belongings as space allows. These sentimental items help to make their room become their home. We just ask that articles or furnishings be cleared through the Director of Housekeeping, Social Services Director, the Director of Nursing, or the Administrator as we must adhere to state regulation and safety codes.Leave of AbsenceWe are most pleased when families can take a resident off the premises for leaves of both short or long durations. All leaves must be scheduled in accordance with the resident's medical ability. If a leave is not medically advisable, the supervisory nurse will advise the family and enter such into the residents medical records.Advance notice is appreciated so the supervisory nurse can arrange for dispensing of medications. Holiday leaves are also encouraged and we will make every effort to accommodate your schedule. We ask if you have a clothing preference to let us know several days before the holiday so staff can plan their schedule in a timely manner. We may ask that you let us know ahead of the holiday when you plan to pick up your loved one and the time of return so we can plan our work flow. Room AssignmentsRoom assignments are made in accordance with availability. We make every effort to see to the compatability of each Resident allowing time for adjustments. Every effort is made to maintain each resident in their own comfortable environment. Due to changing health status, placement in an alternative room may be necessary. Every reasonable effort will be made to contact the responsible party as far ahead as possible, prior to the change. If unable to reach the responsible party, every effort will be made after the move to inform the party of the Residents new location.Physicians ChoiceWe have several qualified physicians who provide care in our facility. Our Medical Directors are Dr. James Chillcott who is located in North Kingsville and the Ashtabula Clinic, and Dr. A.S. Lee located on the west side of Ashtabula. There are many other physicians who provide care within our facility. Some of which include, but are not limited to:
Before admission, residents and/or their responsible party will have their choice of which physician they would like to cover while here. PharmacyPharmed Pharmacy from West Lake, Ohio covers our pharmaceutical needs and supplies. A resident, or thier family is welcome to use a pharmacy of their choice so long as the pharmacy is able to promptly fill medications when needed, and deliver the medications to the nursing home within 24 hours of receiving telephoned and faxed prescriptions.ClothingThe Residents can bring and wear their own clothing. Either the facility, or the family can mark the clothing upon admission. Due to limited space we ask that a reasonable amount of seasonal clothing be brought. Any out of season clothing should be stored at home if possible.Various physical and medical needs may require some limitations as far as some clothing is concerned. Any questions or comments are welcomed and to be directed to the social service representative on the corresponding wing. |
